CHCW’s Connect Program has a full time opening for a Care Coordinator to assist our organization with the improvement and expansion of our Chronic Pain and substance use disorder services focusing on Medication-Assisted Treatment (MAT). This person will work out of our Community Health of Central Washington – Ellensburg Clinic.

The Care Coordinator will perform screenings of program entrants to identify client needs, anticipate challenges and establish goals while working collaboratively with the Connect Program Director and members of our clinical team including providers, nursing staff, pharmacists and behavioral health team to provide education, case management and assessments. Incumbent will coordinate “pain clinic” group visits and classes, perform nursing functions related to 1:1 visits, maintain pain contracts and coordinate patient prescription needs. Will be required to take triage calls and receive undifferentiated referrals, perform initial contact and orientation for referrals into the center’s programs as a result of formal assessment.

BSN preferred/or ADN with at least 2 years clinical experience.  A valid Washington RN or other health professional license with an equivalent health care/behavioral health degree required. Must demonstrate excellent written/oral communication skills including the ability to lead; bilingual E/S a plus.
