CHCW is in search of a full time Care Coordinator Lead to assist the Population Health Manager in managing the overall operations of the Care Coordination/Case Management team at the clinics. Responsibilities include oversight of transitions in care, patient health education for chronic care management, social determinants of health, Health Home patients and coordination of clinical integrated health team. The ideal candidate is an outgoing and self-motivated individual that works collaboratively with the Care Coordination team, Case Management team and clinic staff to provide patient guidance and facilitation of self-management and resource management.
Bachelor’s Degree in Nursing or RN Case Management plus 2 or more years of clinic office nursing experience preferred. Prior experience with chronic disease management of care and patient education preferred. Associate of Nursing Degree with 3-5 years of applicable experience required. Current WA RN License required. Must demonstrate excellent written/oral communication skills and the ability to lead; bilingual E/S preferred.
