CHCW is in search of a part time Care Coordinator to join our Care Coordination Team. The ideal candidate is an outgoing and self-motivated individual that works collaboratively with the Care Coordination team and clinic staff to provide patient guidance and facilitation of self-management and resource management. The Care Coordinator utilizes the core competencies of Assessment, Planning, Facilitation, Monitoring and Advocacy for services to meet the health care needs of our patients.

BSN or ADN with at least 2 years clinical office experience, and/or hospital experience and/or case management, care coordination or chronic disease management. Must demonstrate excellent written and verbal communication skills including the ability to lead; bilingual E/S preferred.
