Community Health of Central Washington (CHCW) is seeking a Case Manager to provide authorization, oversight and monitoring of the client’s plan of care enrolled in Health Home. Case Manager will maintain ongoing contact with clients to enable prompt response to changes in the client’s condition. Case management responsibilities include assessment, care planning and monitoring of client status and implementation and coordination of services.
Qualified applicants will have a Bachelor’s degree in Social Work or Social Sciences and 2 or more years of recent case management experience; preferably in a clinical or hospital setting. Valid driver’s license required. Bilingual E/S preferred.
