CHCW has an opening for a Population Health Manager (PHM) to oversee our Population Health department including RN Care Coordinators, Outreach & Enrollment, Patient Access staff and Community Health Workers. The PHM will work collaboratively with CHCW Leadership and Clinical Directors as well as act as a liaison between CHCW, our neighborhoods and other population and care management partners; including health plans; to improve the health of our communities.
Requirements include the ability to network, develop and maintain working relationships with partners throughout our communities. Successful candidates must be strategic thinkers and planners. A Bachelor’s degree in Healthcare Administration, Public Health or Public Administration and experience in staff supervision is required. Bilingual E/S a plus.
